The Sparta Area School District is working cooperatively with Scenic Bluffs Community Health Centers to provide assistance to those who desire help in navigating through the Wisconsin Healthcare Marketplace. The Marketplace has been set up to provide information on how to apply for health insurance coverage, including Medicaid, BadgerCare Plus, and the Children's Health Insurance Program (CHIP).
The Sparta Area School District is providing open computer lab times for parents to access the Healthcare Marketplace. Scenic Bluffs is providing certified application counselors to assist in the process. Two opportunities are currently scheduled: Wednesday, November 6, 6:30 PM and Thursday, November 7, 10:00 AM. Both will be held at Meadowview Intermediate School, 1225 North Water Street in Lab E117. Both sessions will have someone with Spanish language skills.
Parents planning on coming should have with them all participant social security numbers or document number for legal immigrants, employer and income information for every household member who needs coverage (pay stubs or W-2 forms) and policy numbers for any current health insurance plans covering members of the household. In addition, it will be helpful to have the form "Employee Coverage Tool" completed. This is available at http://marketplace.cms.gov/getofficialresources/publications-and-articles/marketplace-application-checklist.pdf. Copies of this form in all school offices.
